Walk-in appointments:

We always have same day appointments available for urgent needs. Just call ahead of time to schedule an appointment and we will make every attempt to accommodate your needs.

What if I need Emergency Care?

To provide adequately for your emergency medical needs, please contact us by telephone at (541) 779-5531. Your call will be directed to our after-hours service who will assist you with your call and transfer you to a nurse or physician as needed. For life-threatening emergencies (severe difficulty breathing, chest pain, unconsciousness, seizures, or severe head injury) call 911 immediately.

What age range do your physicians treat?
Family Practice Group provides care from newborn to geriatrics. We take pride in providing comprehensive services for every stage of life.

When is my child due for immunizations?

Using our Electronic Medical Records we assist you in keeping your children’s immunizations up to date and will mail you reminders. For details on Jackson and Josephine County school district immunization requirements please visit our immunization web page.

If your child was immunized in Oregon, you can access their immunization record at Oregon Immunization Alert .

How can I get a lab test?

For our patient’s convenience, we offer lab testing services at our office Monday – Friday, 7:30 a.m. to 4:30 p.m. Call for an appointment at (541) 779-5531.

How do I get my lab test results?

You will be notified within two to three days after we receive your results. Family Practice Group is proud to be one of the first family medicine clinics using an electronic medical records system. What that means for our patients is faster test results. Because of direct interfaces with local testing facilities, some lab test results can be received back next day.

Will my insurance pay for the services?

Your insurance card should indicate the name of the group you are insured with. Please refer to our list of contracted insurance providers to see if we are contracted with your insurance provider. Family Practice Group advises patients to contact their insurance as each plan offers different levels of coverage. Please check the back of your wallet card for contact telephone numbers. If we are not contracted with your carrier, you may be responsible for a larger balance than you have experienced in the past.

How can I get a prescription refilled?
Please contact your pharmacy or request a refill through your patient portal.

What if I need to see a specialist?

As a primary care practice, we can take care of most of your health care needs, some care requires referral to a specialist. Some insurance companies require you to have a formal referral in place before you make your appointment with the specialist..
