Health Care Insurance Coverage
Family Practice Group is contracted with most major insurance carriers. Your insurance card should indicate the name of the group you are insured with. Family Practice Group advises patients to contact their insurance carriers as each plan offers different levels of coverage. Please check the back of your wallet card for contact telephone numbers.
It is your responsibility to know the requirements of your own insurance. We will do our best to help you with any questions you may have, but ultimately you are responsible for any co-payments and/or deductibles required by your insurance.
Patients with a non-participating insurance must pay at the time of the service. Check your individual plan for any special requirements concerning second opinions or pre-authorizations for surgical procedures, etc.
Please refer to our list of contracted insurance providers below to see if we are contracted with your insurance provider. If we are not contracted with your carrier, you may be responsible for a larger balance than you have experienced in the past.
